     为了鼓励石材http://www.stonebuy.com/生产企业http://www.stonebuy.com/enterprise/和石材http://www.stonebuy.com/装饰企业http://www.stonebuy.com/enterprise/加强质量管理，增强质量意识，提高石材http://www.stonebuy.com/建筑装饰工程整体质量水平，鼓励建筑装饰企业http://www.stonebuy.com/enterprise/争创石材http://www.stonebuy.com/装饰名优工程，中国http://www.stonebuy.com石材http://www.stonebuy.com/工业协会自2005年设立“华表杯”优质石材http://www.stonebuy.com/装饰工程奖，经过企业http://www.stonebuy.com/enterprise/申报，专家评审，媒体公示等程序，共有8个工程项目获得首届“华表杯”优质石材http://www.stonebuy.com/装饰工程奖，在行业内外引起较大反响。

     根据《华表杯优质石材http://www.stonebuy.com/装饰工程评选暂行管理办法》规定，现决定在全国范围内开展第二届“华表杯”优质石材http://www.stonebuy.com/装饰工程评选活动，现将有关事项通知如下：www.stonebuy.com

     一、申报评选范围：生产、施工建筑装饰企业http://www.stonebuy.com/enterprise/及工程业主，符合以下范围和条件的建筑实物，均可申报参加评选。
　　1．中国http://www.stonebuy.com境内的应用石材http://www.stonebuy.com/装饰装修的各种建筑物及公共场所和设施；
　　2．工程设计符合国家和部颁建筑装饰设计有关标准、规范的要求；
　　3．工程施工质量达到国家建筑工程质量检验评定标准的优良等级；
　　4．所选用的装饰石材http://www.stonebuy.com/必须经省级以上质检部门检验合格；
　　5．独家承担单项（包括：新建、翻建、改建、改造）石材http://www.stonebuy.com/装饰工程面积在5000平方米以上；
　　6．申报项目必须是2005年1月1日年至2006年12月31日之间已取得竣工验收证书的工程项目；
　　7．项目验收后至今没有发生质量缺陷的工程项目；
　　8．工程材料及施工技术档案资料齐全、完整、有效。

     二、评选办法及程序
　　1．企业http://www.stonebuy.com/enterprise/填写申报表格，申请一项填写一张表格；
　　2．企业http://www.stonebuy.com/enterprise/备齐申报项目应提交的文件资料，连同申报表一起寄送到中国http://www.stonebuy.com石材http://www.stonebuy.com/工业协会；
　　3．中国http://www.stonebuy.com石材http://www.stonebuy.com/工业协会秘书处收到企业http://www.stonebuy.com/enterprise/资料后，分发到评委会每个委员手中，进行审阅；
　　4．集中评委意见，初评出候选工程，并在《石材http://www.stonebuy.com/》杂志和中国http://www.stonebuy.com建材报、中国http://www.stonebuy.com石材http://www.stonebuy.com/之窗网站上进行公示，征求业内外人士的意见；news.stonebuy.com
　　5．汇总反馈意见，提交评委会研究讨论，最终确定获奖工程项目。

     三、申报项目应提交的文件材料
　　1．如实填写《“华表杯”优质石材http://www.stonebuy.com/装饰工程申报表》；
　　2．申报企业http://www.stonebuy.com/enterprise/营业执照副本复印件，加盖公章；
　　3．石材http://www.stonebuy.com/企业http://www.stonebuy.com/enterprise/与建筑甲方签订的供货协议（合同）复印件，加盖企业http://www.stonebuy.com/enterprise/公章；
　　4．申报工程项目的设计说明，施工总结及工程竣工后七寸彩色全景照片四张或工程相关图像资料光盘一张；
　　5．施工期间工程用石材http://www.stonebuy.com/质检报告（必须提供抽样检测报告）；
　　6．工程项目施工组织方案、保证质量措施；
　　7．工程项目质量检验评定记录；质量评定表；
　　8．工程项目竣工验收证明、工程质量监督机构质量核定文件；
　　9．申报项目整体或分项获得的省部级以上各种奖项奖状及证明文件；
　　10．工程业主给予的评价及相关意见；
　　11．工程项目所在地区业务主管部门签署意见。

     四、企业http://www.stonebuy.com/enterprise/申报时间截止到2008年1月31日
　　评审时间：2008年1月31日— 2月28日；
　　公布初选结果时间：2008年3月初；
　　公布最终评选结果时间：2008年3月31日。

     五、其他事项：
　　1．“华表杯”优质石材http://www.stonebuy.com/装饰工程评选是一项公益事业，服务于国内整个石材http://www.stonebuy.com/行业，评选活动不收取任何费用；
　　2．评选活动遵循公平公正原则，评委会人员组成事先不对外公布，不允许申报单位人员与评委直接接触。
　　3．获得“华表杯”优质石材http://www.stonebuy.com/装饰工程奖的工程项目由中国http://www.stonebuy.com石材http://www.stonebuy.com/工业协会发文表彰，并在相关媒体上报道宣传，适当时机召开表彰大会，颁发获奖证书、奖牌和奖杯。pic.stonebuy.com
