关于饰面石材备案管理相关事项的通知

为了加强建设工程的安全质量管理，提升石材http://news.stonebuy.com/企业http://www.stonebuy.com/enterprise/的能力与水平，同时，作为上海石材http://news.stonebuy.com/业纳入政府规范管理的轨道，上海石材http://news.stonebuy.com/行业协会协助上海市建筑建材业受理服务中心，进一步做好饰面石材http://news.stonebuy.com/备案管理工作，相关事项如下：
一、根据《上海市建设工程材料管理条例》及市建委《关于印发<上海市建设工程材料使用监督管理规定>的通知》等相关文件规定，对用于本市建设工程的饰面石材http://news.stonebuy.com/实施备案管理，石材http://news.stonebuy.com/供应商根据企业http://www.stonebuy.com/enterprise/注册地到上海市建筑建材业受理服务中心及各区县建材质监部门办理备案手续，未按规定办理备案手续的企业http://www.stonebuy.com/enterprise/石材http://news.stonebuy.com/不得用于本市建设工程。www.stonebuy.com
1、 从事石材http://news.stonebuy.com/加工、贸易http://www.stonebuy.com/trade/及工程，50万以上注册资金；
2、 具备相应的石材http://news.stonebuy.com/加工设备；
分毛板加工、饰面板（工程板）加工、异型板加工三种，设备要求：

1、 填写备案表（协会提供）
2、 企业http://www.stonebuy.com/enterprise/营业执照原件与复印件
3、 质量承诺书（样式向协会索取）
4、 协会评估报告
5、 如果初次备案，提供一年内有效的大理石物化、花岗石物化及放射性检测报告；如是有效期满年度登记，则不需提供产品http://www.stonebuy.com/sample/检测报告。
四、上海石材http://news.stonebuy.com/行业协会负责对石材http://news.stonebuy.com/备案工作的咨询辅导及能力评估工作，并根据要求出具评估报告。
五、协会对备案企业http://www.stonebuy.com/enterprise/设立诚信档案，进行诚信管理，每年进行一次年度能力评估；市建筑建材业受理服务中心对备案企业http://www.stonebuy.com/enterprise/在每二年有效期满，根据综合评估报告进行年度重新登记。
六、所有备案企业http://www.stonebuy.com/enterprise/以及2006年新备案企业http://www.stonebuy.com/enterprise/将统一于2006年12月31日到期进行年度登记。对于2006年新备案企业http://www.stonebuy.com/enterprise/如无诚信不良记录，将予以直接进行年度登记。
七、所有石材http://news.stonebuy.com/备案企业http://www.stonebuy.com/enterprise/将于2006年12月换证时统一领取IC卡及质量诚信手册，协会对备案企业http://www.stonebuy.com/enterprise/依据行业自律原则进行日常诚信管理，综合评估未达标的将不予年度登记，诚信管理记录进入上海市建筑建材业管理体系。
八、上海石材http://news.stonebuy.com/行业协会将配合市建委相关部门，加强对备案企业http://www.stonebuy.com/enterprise/的日常监督，加强对未备案企业http://www.stonebuy.com/enterprise/的查处。

1、上海石材http://news.stonebuy.com/行业协会接受石材http://news.stonebuy.com/企业http://www.stonebuy.com/enterprise/委托，对其石材http://news.stonebuy.com/加工和工程供料能力进行评估，并对评估结果的有效性负责。news.stonebuy.com
2、评估程序是先由企业http://www.stonebuy.com/enterprise/根据要求提供书面资料，在对书面资料初步核实并满足备案要求的前提下，派评估专家赴企业http://www.stonebuy.com/enterprise/现场核实评估。
3、评估结果以石材http://news.stonebuy.com/企业http://www.stonebuy.com/enterprise/提供的资料及现场加工情况的真实性为前提，否则评估报告自动失效，并将保留追究企业http://www.stonebuy.com/enterprise/相应的法律责任的权力。
4、评估企业http://www.stonebuy.com/enterprise/必须提供企业http://www.stonebuy.com/enterprise/营业执照、上年度财务报表的原始资料和复印件，原件经审核无误后当场退还，以及从业人员数量和结构，年加工能力、厂区及厂房面积、加工设备、质量管理等书面资料，评估企业http://www.stonebuy.com/enterprise/应配合评估专家根据要求提供进一步的资料。
5、企业http://www.stonebuy.com/enterprise/基本情况如注册资金、固定资产、年加工能力、厂区及厂房面积、从业人员数量及结构、石材http://news.stonebuy.com/加工设备等的评估根据企业http://www.stonebuy.com/enterprise/提供的相关资料原件并现场核实为准。
6、石材http://news.stonebuy.com/企业http://www.stonebuy.com/enterprise/质量管理评估分为质量管理规章制度(总分10分，合格6分) 、技术与管理专业人员(总分10分，合格6分) 、产品http://www.stonebuy.com/sample/质量检测设备(总分10分，合格6分) 、质量管理实施情况(总分10分，合格6分)4个方面进行评估，4项累加分24以下为不合格，24-40分为合格。
7、石材http://news.stonebuy.com/能力评估合格的基本条件：
a) 企业http://www.stonebuy.com/enterprise/注册资金50万元以上
b) 企业http://www.stonebuy.com/enterprise/加工厂区面积或厂房建筑面积200平方米以上
c) 企业http://www.stonebuy.com/enterprise/从业人员20人以上，其中石材http://news.stonebuy.com/加工工人10人以上（具备上海石材http://news.stonebuy.com/行业协会颁发的上岗证书）
d) 具有相应的石材http://news.stonebuy.com/专业技术人员（具备市紧缺人才办颁发的石材http://news.stonebuy.com/岗位资格证书）
注册资金50-100万：一名加工师，一名项目经理
注册资金100-500万：二名加工师，二名项目经理
注册资金500万以上：三名加工师，三名项目经理
e) 企业http://www.stonebuy.com/enterprise/质量管理评估为合格，无不良诚信记录。
f) 企业http://www.stonebuy.com/enterprise/产权拥有或租赁使用权（1年以上）拥有如下设备
